In 1928, a scientist named Alexander Fleming forgot to clean his lab before vacation. When he came back, he noticed something strange: a blue-green mold had grown on a petri dish โ and every bacterium around it was dead. That mold produced penicillin, the world's first antibiotic. For decades, antibiotics were miracle medicines. But today, more than 1.2 million people die every year because antibiotics no longer work on certain bacteria. Why? Because bacteria evolved to fight back.

What Are Antibiotics?
Antibiotics are medicines that kill bacteria or stop them from reproducing. They do NOT work on viruses โ so if you have a cold or the flu, antibiotics won't help. Different antibiotics attack bacteria in different ways: - Penicillin and amoxicillin break apart the outer wall of bacteria, like poking holes in a water balloon until it pops - Tetracyclines block bacteria from making the proteins they need to survive - Fluoroquinolones jam the machinery bacteria use to copy their DNA Each method targets something bacteria need to live. The problem? Bacteria are incredibly good at adapting.
How Bacteria Become Resistant
Imagine a population of 10 million bacteria inside your body. Most are identical, but a few โ maybe 100 of them โ carry a random mutation that helps them survive a particular antibiotic. When you take that antibiotic: 1. The 9,999,900 normal bacteria die 2. The 100 resistant bacteria survive and keep reproducing 3. Within hours, all the bacteria in your body are descendants of those 100 resistant survivors 4. The antibiotic no longer works This is natural selection happening in real time. Bacteria reproduce so fast โ some double every 20 minutes โ that evolution can happen in a single day. Resistance also spreads between bacteria through horizontal gene transfer โ bacteria can actually pass resistance genes directly to each other, like sharing a cheat code.
When you take antibiotics, your doctor gives you a full course โ say, 10 days. Many people feel better after 5 days and stop taking the medicine. This is dangerous! The strongest, most resistant bacteria survive until the very end of treatment. Stopping early lets them live and reproduce, creating a harder-to-kill population. Always finish your full course of antibiotics.
Real Bacteria That Resist Our Best Medicines
Antibiotic resistance is not a future problem โ it is happening right now. MRSA (Methicillin-Resistant Staphylococcus aureus) causes skin infections and pneumonia. It resists methicillin and many other antibiotics. In the US, MRSA kills about 19,000 people per year โ more than HIV/AIDS. Tuberculosis (TB) bacteria have developed resistance to isoniazid and rifampin, the two most important TB drugs. Multi-drug resistant TB requires treatment lasting 18-24 months and is much harder to cure. C. diff (Clostridioides difficile) causes severe diarrhea, often after patients take antibiotics that kill their normal gut bacteria. It causes about 30,000 deaths per year in the US.
In many countries, antibiotics are prescribed far too often โ for ear infections that would heal on their own, for colds caused by viruses, or because patients ask for them. Every unnecessary prescription is another chance for bacteria to evolve resistance. Some doctors now practice watchful waiting for minor infections, prescribing antibiotics only if things get worse.
